Most don't make the dresses themselves, there are specialized shops in Amsterdam and Germany for instance where you can make an appointment and they measure you up so the dress fits perfectly, you can choose the style, material, color and so on yourself. After a few weeks you can pick the dress up. These are very specialized shops and in many cases you have to make an appointment a few months in advance. The materials used are of course the finest you can find, that's also the part why they are so expensive.

No, I've never been to one of those, and to the $1500 dresses, most wedding and prom dresses are also extremely expensive.
